Shoulder Structure

The shoulder is a complex joint formed by three bones: the collarbone (clavicle), the shoulder blade (scapula), and the upper arm bone (humerus). The shoulder blade's acromion forms the top of the shoulder, creating a structure that supports a wide range of movements thanks to its four separate joints.

The main joint, known as the ball-and-socket or glenohumeral joint, features the humerus' "ball" fitting into the "socket" of the scapula, called the glenoid. This socket is shallow compared to other joints like the hip, which means it relies heavily on surrounding structures for stability. A special tissue called the labrum deepens this shallow socket, enhancing stability, especially during upward movements like throwing. The rotator cuff, comprising four tendons connected by muscles from the shoulder blade, also plays a crucial role in movement and stability.

Unlike the protected hip joint, the shoulder's openness allows for greater movement but also makes it prone to dislocations—the most common among major joints. Additionally, between the rotator cuff and the acromion, there are small fluid-filled sacs called bursae that cushion and facilitate smooth movements of these tendons over the bone.

Shoulder problems often stem not from the bones but from the muscles, ligaments, and tendons, exacerbated by activities that require repetitive overhead actions, such as swimming, tennis, and even daily tasks like painting or gardening. For diagnosing these issues accurately, an arthroscope—a specialized diagnostic tool—is typically used.

IMPINGEMENT SYNDROME

Impingement syndrome represents the initial phase of possible issues with the rotator cuff. Often caused by repetitive motions or the natural degeneration associated with aging, this condition can lead to inflammation of the rotator cuff or biceps tendons, known as tendinitis. Additionally, it might irritate the bursa—a fluid-filled sac that serves as a cushion between the top of the shoulder blade and the rotator cuff, facilitating the smooth movements of the tendon.

This syndrome is termed "impingement" because the tendons of the rotator cuff are compressed or pinched between the acromion and the moving head of the upper arm bone, leading to inflammation and swelling of these tendons.

Treatment

For many individuals, tendinitis and bursitis can be effectively managed with rest, ice application, and anti-inflammatory medications, often supplemented by physical therapy or additional medications. However, if these conservative approaches fail to alleviate the condition, arthroscopic surgery may be recommended. This surgical procedure aims to enhance the mobility of the rotator cuff tendons by enlarging their space. It typically involves the removal of part of the acromion bone's lower surface and the excision of inflamed bursal tissue to allow for unimpeded tendon movement.
